Job Title: Aged Care Facility Coordinator

Your Opportunity

Operating from within the Health Services Department, you will be responsible for providing holistic resident-centred care to residents within the Western Downs Regional Council Aged Care Facility assigned.

Key Responsibilities

Use your well-developed management skills to:

  • Coordinate the operations of the Aged Care facility the Officer is assigned to;
  • Monitor and review the staffing structure in consultation with the Health Services Manager to ensure it is appropriate in meeting the organisational goals for residential care provision in a dynamic environment of care needs;
  • Develop and maintain communication processes which effectively support the strategic direction of the site by ensuring information is provided which enables all staff to perform their roles effectively;
  • Maintain and actively contribute to an environment which facilitates and encourages continuous improvement, responsibility, economy and accountability;
  • Guide processes which facilitate achievement of compliance across all standards of accreditation;
  • Promote education and workplace performance that supports safe workplace practices and ensures all staff meet mandatory training requirements;
  • Report and review workplace incidents and assess and manage risks;
  • Ensure Workplace Health and Safety processes are followed in accordance with Western Downs Regional Council policies and procedures.

Use your high-level clinical skills:

  • Assess residents' care needs in a holistic manner and ensure care plans are appropriate to their needs and developed in consultation with the resident and/or their legal representative;
  • Ensure systems are in place so that the regular and timely monitoring and review of the care provided enables an individual approach;
  • Collate and analyse clinical indicators in a timely manner in order to direct appropriate responses to correcting adverse trends;
  • Ensure Aged Care Funding Instrument (ACFI) claims are supported by accurate assessments and resident data, and that claims monitoring occurs to ensure staff knowledge facilitates the maximisation of funding within the legislative framework.

Use your public relations skills:

  • Provide assistance to prospective residents and their families in their deliberation regarding selection of the most appropriate residential care to suit their particular requirements. It is important that this information is provided in a consistent and helpful manner;
  • Ensure feedback from residents and/or relatives is managed in a way which ensures an environment free from recrimination where

    complaints are accepted as an opportunity for improvement;

  • Customer focus (e.g. Active listening, delivery and follow-up) responsive to individual, facility and organisational needs;
